Whether it is a neighbor without HEALTH insurance, a victim of abuse, or someone struggling with mental illness or an addiction, the United Way is working to help everyone have access to affordable and quality care. The following is our Community Impact Plan for HEALTH.

VISION: Empowering citizens of our community with personal responsibility for their health and well-being through personal health awareness and increased access to healthcare and wellness resources.

  1. BARRIER: Health care services and wellness opportunities are not readily available or affordable to everyone in the community.
    1. COMMUNITY LEVEL OUTCOME: People increase their use of effective primary, behavioral and dental health care regardless of ability to pay.
      • INDICATOR: There is increased access to affordable medications.
      • INDICATOR: There is increased access to substance abuse treatment.
      • INDICATOR: There is increased access to mental and emotional health services.
      • INDICATOR: There is increased access to dental services.

         

  2. BARRIER: Citizens may not take personal responsibility for health care services and wellness opportunities. 
    1. COMMUNITY LEVEL OUTCOME: People have increased awareness for wellness and prevention.
      • INDICATOR: There is an increase in participation in preventative health and wellness education and training.
      • INDICATOR: Individuals and families increase understanding of their health condition(s) and risks.
      • INDICATOR: Individuals and families will improve and maintain their physical health.

         

  3. BARRIER: Many citizens cannot take advantage of healthcare or wellness opportunities due to lack of reliable transportation.
    1. COMMUNITY LEVEL OUTCOME: People have increased awareness of transportation options and utilize those opportunities for wellness and prevention.
      • INDICATOR: There is an increase in participation in preventative health and wellness education and training and meeting appointments.
      • INDICATOR: There is improved communication and linkage between service providers and public transportation entities.
